I can confirm that I have never been refused registration or removed from any other professional register and I am free of any circumstances that might affect my fitness to practise.

I am aware that should I work outside of the UK that I will not be covered by the Society’s insurance unless I have a special arrangement in place and will therefore need to seek additional cover within the countries that I practise.

I confirm I have adequate, current and ongoing professional indemnity insurance for any additional therapies I practice and not covered by the Society’s insurance.

I confirm that I have not been subject to any disciplinary proceedings and know of no complaint, and no circumstance which could reasonably be expected to result in a complaint, no legal action, civil or criminal, and no threat of legal action alleging negligence, error, or omission against me.

I understand I am required to inform the Society of Homeopaths if I hold any offences which are currently unspent under the Rehabilitation of Offenders Act 1974. Failure to do so will mean immediate termination of my membership with the Society of Homeopaths.
